Data Agreements

After the request is approved, submit an approval letter from your IRB along with the appropriate agreement data agreement. If you are requesting a limited data set, you need both the Data use agreement and the Data use addendum. If you are requesting de-identified data, you need only the Data distribution agreement.  Print, sign, and mail the agreement(s) to Dr. Daly.
